Important Information To File a Workers Compensation Claim

When you decide to file a workers compensation claim it is helpful to provide the following information to insure the claim is filed correctly:
1. Pay Stub
◦ A pay stub will provide the most accurate name and address of you employer;
◦ Accurate wage statement;
2. Copy or photo of the workers compensation Panel of Physicians;
3. Site address where the injury occurred – ReisLaw can often pull permits or find the correct parties if the immediate employer is uninsured. This also helps determine the correct party to bring the claim;
4. List of all medical providers;
5. List of any previous injuries, body parts injured and medical providers for those injuries;
6. Adjuster Information – If the claim is already set up you will need the adjuster name, address and phone number so the Attorney can call them directly
